▲During pregnancy, those with a history of heart disease or elevated blood pressure need to consult physicians with expertise in both obstetrics and cardiology, who can perform specialized cardiac ultrasound techniques. By Dr. Cheng Kai-hung

In her early thirties, Xiao Qi often felt chest discomfort while lying down, which prompted her to seek medical attention from various doctors. Despite having a heart murmur and some foot swelling, previous examinations hadn't revealed anything noteworthy. However, when she underwent a cardiac ultrasound, including a special advanced 4D strain echocardiogram, it was found that although she was young, the motion of her heart's posterior and lateral walls during contraction was somewhat impaired, particularly after giving birth just one month prior.

I asked her, "Did your blood pressure increase during pregnancy?"

She replied, "The systolic pressure went up by over 20 millimeters of mercury (mmHg) compared to before pregnancy."

I responded, "I see! I'll prescribe medication for you, but keep in mind that the medication isn't suitable for breastfeeding."

◎ Understanding Obstetric Cardiology:

● Obstetric cardiology is an emerging interdisciplinary field that combines cardiology and obstetrics. Physicians in this field pay special attention to cardiovascular diseases that arise or worsen during pregnancy and childbirth, as well as postpartum cardiovascular health. Failure to promptly address these conditions can result in severe complications for pregnant and postpartum women.

● Peripartum cardiomyopathy commonly occurs during the last month of pregnancy or within six months after delivery. Symptoms include palpitations, chest pain, shortness of breath, fatigue, edema, and arrhythmias. Left untreated, it can lead to severe heart failure and other complications.

● Gestational hypertension refers to high blood pressure that develops in pregnant women after the 20th week of pregnancy. If accompanied by proteinuria or other kidney abnormalities, it may indicate the possibility of preeclampsia or eclampsia.

Although gestational hypertension typically resolves after pregnancy, in certain cases, it can progress to more severe conditions like the aforementioned disorders or peripartum cardiomyopathy. During pregnancy, it is advisable to follow medical advice, undergo regular check-ups, and use relevant medications cautiously to avoid affecting the baby.

Early Detection Prevents Serious Complications

For those with a history of heart disease or elevated blood pressure during pregnancy, it's important to consult physicians with expertise in obstetric cardiology and who are skilled in performing specialized cardiac ultrasound techniques. They may need more time to assess your heart health to ensure early detection of any issues.

If you experience symptoms like palpitations, chest pain, shortness of breath, fatigue, swelling, or other discomforts, seek medical attention promptly. Early detection and treatment are the best ways to safeguard your health and the health of your baby.
