一、病人權利 Patient Rights

1.平等權:本院對所有病人一視同仁,不因種族、年齡、價值觀與信仰而有差別待遇。 Right to Equality: Our hospital treats all patients equally without discrimination based on race, age, values, or beliefs.

2.專業安全權:您有權知道照護人員姓名、職稱與服務內容;若未佩戴識別證,您可拒絕其醫療服務。 Right to Professional Safety: You have the right to know the name, title, and role of your care providers; you may refuse services from anyone without proper identification.

3.知悉權:您有權了解病情、診斷、治療計畫、預後及藥物相關資訊,並可向醫護人員提問。 Right to Information: You have the right to understand your condition, diagnosis, treatment plan, prognosis, and medication details, and to ask healthcare providers for clarification.

4.同意權:侵襲性檢查或治療前,醫師會先說明風險並取得您或家屬的書面同意後才進行。 Right to Consent: Before any invasive test or treatment, your physician will explain the risks and obtain written consent from you or your authorized representative.

5.選擇權與拒絕權:您可自由選擇是否接受醫師所建議的檢查或治療。 Right to Choose or Refuse: You have the right to accept or decline any examination or treatment recommended by your physician.

**6.申請權:**您或授權人士可申請個人證明文件、檢查報告影本、影像複製、診斷證明、病歷摘要等。 Right to Access Information: You or your authorized representative may request medical certificates, test report copies, imaging records, diagnosis statements, and medical summaries.

**7.隱私權:**本院尊重並保護您的隱私,依法對您的病情及就醫資料負保密義務。 Right to Privacy: We respect and protect your privacy and maintain confidentiality regarding your condition and medical information.

**8.申訴權:**若您對醫療或服務不滿,可透過本院的多項管道提出申訴。 Right to Complaint: If you have concerns or dissatisfaction, you may file a complaint through our available channels.

二、病人責任 Patient Responsibilities

**1.誠實告知:**請誠實告知病情、病史及過敏史,以利提供適切醫療。 Honest Disclosure: Please provide accurate information regarding your condition, medical history, and allergies to ensure proper care.

**2.知情同意:**在決策前請了解治療的風險,並對簽署的同意書及您的醫療選擇負責。 Informed Consent: Understand the risks of treatment before making decisions and take responsibility for any consent forms you sign.

**3.積極配合:**請配合醫護人員的醫療計畫;若無法接受,請主動說明原因以利調整。 Active Cooperation: Please follow the medical plan provided; if you cannot comply, inform staff so alternative arrangements can be made.

**4.確認個人資料:**請核對您的個人資訊與藥品內容,並確保醫療人員已確認您的身分。 Verify Personal Information: Please check your personal data and medications, and ensure staff verify your identity before treatment.

**5.立即詢問:**若對治療或健康指導不清楚,請立即詢問醫護人員。 Ask When Unsure: If you do not understand your medical plan or instructions, ask healthcare staff promptly.

6.真實身份就診:請遵守法令,不得要求開立不實診斷或提供不實資料。 Use True Identity: Follow legal and hospital regulations and do not request false documentation or diagnoses.

7.不亂服用藥品:若使用非本院藥品,請主動告知醫護人員。 Avoid Self-Medication: Inform healthcare staff if you are taking medications not provided by our hospital
