Standard Operating Procedure for Bowel Preparation in Patients with Impaired Renal Function
1. 腎功能差定義：eGFR ＜ 30 mL/min/1.73m² 或 CCr ＜ 30 mL/min
Definition of impaired renal function: eGFR < 30 mL/min/1.73m² or CCr < 30 mL/min.
2. 院內可選擇清腸劑共兩種：Bowklean 保可淨、Niflec 耐福力散
Two bowel preparation agents are available in the hospital: Bowklean and Niflec.
腸胃科醫師依據 eGFR 或 CCr 開立適當清腸劑。
Gastroenterologists prescribe the appropriate bowel preparation based on the patient’s eGFR or CCr.

✔ 建議使用 Niflec 耐福力散
✔ Niflec is recommended.
✔ 醫師判斷腎功能是否適合使用
✔ The physician determines whether the renal function is adequate for use.
檢查前兩週內需進行腎功能檢驗：檢測 eGFR 或 CCr
Renal function tests (eGFR or CCr) must be performed within two weeks before the procedure.
清腸劑使用衛教
Education on the use of bowel preparation agents
由藥師及健康管理師提供清腸劑使用衛教。
Pharmacists and health managers provide education on bowel preparation instructions.
※ 腎功能較差之貴賓須確實補充水分 1500–2000 c.c.
※ Patients with impaired renal function should maintain adequate hydration of 1500–2000 c.c.
若使用清腸劑有任何疑慮，皆可由藥師、健康管理師協助。
If there are any concerns regarding the bowel preparation, pharmacists or health managers can provide assistance.
受檢者依預約日期進行胃腸內視鏡檢查。
The patient undergoes the gastrointestinal endoscopy on the scheduled date.
MRI 檢查注意事項（腎功能不足之受檢者）
MRI Examination Precautions for Patients with Impaired Renal Function
注射含釓對比劑後，其代謝主要由腎臟排出。
Gadolinium-based contrast agents are mainly excreted through the kidneys.
正常腎功能者約於 24 小時內排出體外。
Patients with normal renal function typically eliminate the agent within 24 hours.
若腎功能不佳，可能因排泄不良導致體內滯留，引發組織纖維化病變，發生率約 3〜5％。
If renal function is impaired, poor excretion may cause gadolinium retention, leading to tissue fibrosis, with an incidence of approximately 3–5%.
對於需透析者，建議安排 MRI 於「透析前」進行，以降低對比劑滯留風險。
For patients requiring dialysis, MRI is recommended before the dialysis session to reduce the risk of contrast retention.
